Soldier Readiness Program

Services:

  • IMR (Individual Medical Readiness), PHA, Pre-Deployment DD 2795, Post-Deployment DD 2796, Post Deployment Health Reassessment (PDHRA) DD 2900, POR (Preparation for Overseas-Reassignment) for Army, Marines, Navy and Air Force, DA 5118’s, Marine Packets for Recruiter, Drill Instructor, Martial Arts, and Re-enlistment; Medical Warning Tags, DA 7708 (background checks), Vision Screenings, Immunizations for Medical Readiness.
  • ​Screening for the following is completed at the SRP but will be conducted in other areas of the hospital: ANAM testing, Labs and Hearing

Army Hearing Program/Hearing Conservation

Mission:

  • The Fort Leonard Wood Hearing Program strives to prevent hearing loss and enhance communication for noise exposed personnel. The program has four major elements: hearing readiness, clinical hearing services, operational hearing services, and hearing conservation. Good hearing is essential for a Soldier’s SURVIVABILITY and LETHALITY in combat. Noise induced hearing loss, on the other hand, has significant negative personal, financial, and mission impact on our Soldiers, civilian employees, and the Army.

Services:

  • Walk-ins welcome.  Hearing tests provided for Active Duty Service Members and designated Federal employees for purposes of completing Physical Health Assessments and updating MEDPROS (annual, pre/post-deployment, and physicals). Soldiers should bring their hearing protection with them to complete a fit check. 
  • Initial hearing tests
  • Annual Hearing Tests (Military and noise-exposed civilian workers)
  • Hearing Protection Fittings (Earplugs)
  • Hearing Conservation Officer Training
