Immunizations
Services
We provide immunizations as follows:
- Ages two weeks and up with well-child visits conducted in the past 6 months.
- We request that you bring vaccination records with you to the immunizations clinic if we have not seen you in clinic before.
- Patients are seen by a walk-in basis for immunizations Monday through Friday from 7:30 a.m. - 4 p.m.
- Only those with a Primary Care Manager at the Patient Centered Medical Home are eligible for immunizations at GLWCH.
