Patient Administration Division (PAD)
In order to maintain proper maintenance and documentation of continuity of care, all medical records of active duty, retirees, and their family members must be turned in upon arrival to Fort Leonard Wood, MO. Both in-processing and out-processing are done Monday through Friday from 7:30 a.m.-4 p.m. hours in the Patient Registration Office on the 1st floor, Rm 122 until further notice. You must bring a copy of your orders with you to properly in process. ALL service members must be in uniform to conduct business.
Admissions & Dispositions
The Admissions Office assists patients with completing pre-admission paperwork in anticipation of a birth or surgery. It also serves as the point of contact for decedent paperwork and the facility’s patient directory.

Birth Registrar
The Birth Registrar assists parents with obtaining necessary government documents for births occurring at General Leonard Wood Army Community Hospital.

Medical Records/Release of Information
Download
release of information consent form (DD2870) and email to: Usarmy.leonardwood.medcom-glwach.list.roi@health.mil, or you may fax form (DD 2870) to 573-329-0888 or feel free to visit our office currently located on the 1st Floor, Rm 163. The hours of operation for the Release of Information (ROI) are Monday-Thursday 0730-1700 and Friday 0730-1600, closed on all federal holidays. You are authorized one free complete copy of your medical records. Each additional full copy per year must be paid for at the facility. Please send in a copy of a valid photo ID with your request form or your request will not be processed. Please note that records requests may take up to 30 business days. If you are requesting any radiology imaging this must be completed through our radiology department who can be contacted by phone 573-596-0029.

Medical Record Reminders
Patients receiving treatment at Fort Leonard Wood are no longer permitted to hand carry or maintain their medical records unless the patient is USMC hand carry permitted for EAS out-processing. Marines are not authorized to hand carry their dependent records per regulation DOD 6025.18R.
